Specifically for compassionate use permits, during the affected period you may use the following forms instead:

Human

• Individual: Application for compassionate use permit – Individual Human (docx) (in Danish only)
• General: Application for compassionate use permit – General Human (docx) (in Danish only)

Veterinary

• Individual: Application for compassionate use permit – Individual Veterinary (docx) (in Danish only)
• General: Application for compassionate use permit – General Veterinary (docx) (in Danish only)

Please print the form, scan it, and send it to the Danish Medicines Agency at udleveringstilladelser@dkma.dk (encrypted emails can be received).

If submitting the application via email, please attach the scanned form as a single consolidated file.
